As a licensed physical therapist and certified canine rehabilitation therapist, I provide services under Washington state guidelines in collaboration with veterinarians. A veterinary referral is required before beginning treatment to ensure a valid veterinary-client-patient relationship (VCPR) and to support safe, coordinated care. Referrals can include discharge notes, a completed referral form, or other documentation recommending rehabilitation. I do not diagnose or prescribe medications and work strictly within the scope of my professional training and Washington law.

The referral form below can be filled out online or printed and emailed to info@southsoundk9rehab.com. If you’re emailing the client’s medical record, the referral form only needs the client’s information, diagnosis, and provider signature. Use of our specific referral form is helpful, but not required.
